Instead of a physician reaming him out with a series of sounds, the physician prescribes antibiotics at worst a painful intramuscular injection or three in the buttocks (shot in the ass.)  Men have discovered that the penis can not only be massaged on the outside for pleasure, but also from the inside by inserting objects up the urethra. So the physician's sounds have found a new life. (They still have very limited medical use when the now rare cases that plumbing tightens up.)
