Digital examination is the use of a single digit (finger) for palpation. Specifically, it's what your doctor does when he has you "bend over and spread your cheeks" for the old finger up the rectum check. In this case he is checking the prostate for enlargement and bumps that may be a sign of cancer or other problems. He exits he rotates his hand so that his finger scans the rectum for signs of problems.
